Evaluation methods & forms for different course formats

In case of a EUREPA accredited or ILAE/CEA supported course, an evaluation of the outcome is mandatory, and the results of the outcome must be sent to EUREPA and the respective ILAE Commission or Academy within 3 months after the course.

The evaluation form must include the following question:
“Was this course promotional for any commercial product or company? Yes/No. 
If yes, please provide details: "

In the following you will find different forms for the assessment of the outcome of your activity. The questionnaires listed are only examples and should be adapted to your specific event.

• The evaluation form course [MS Word .doc] is a rather short general form suitable for all kinds of courses.
  
• The evaluation form for courses with many speakers [MS Word .doc] is a version of the general evaluation (above) adapted to courses with many lecturers.
  
• The detailed evaluation form for intensive workshops [MS Word .doc] offers a more detailed evaluation with many questions regarding the opinion on the program and the fulfillment of personal learning goals, suited  for longer lasting and more intensive seminars.
  
  
• For summer school type courses (4-14 day long intensive courses in a retreat setting with 30-40 participants and many lecturers and tutors), we can recommend a combination of two different forms:

They can be used as interim (daily) or final evaluations and include the elements of visualization of the results on moderation walls or flipcharts. These methods are most effective in intensive workshop settings in smaller groups (max. 20) with much personal involvement.

The feed-back form on short interactive sessions [DOC,162KB]  using smileys is a very short visually attractive evaluation form.

The post-evaluation form [DOC,164KB] can be used to assess longterm outcome a certain time after the course.
