Evaluation methods & forms for different course formats


In case of a EUREPA accredited or ILAE/CEA supported course, an evaluation of the outcome is mandatory, and the results of the outcome must be sent to EUREPA and the respective ILAE Commission or Academy within 3 months after the course.

The evaluation form must include the following question:
“Was this course promotional for any commercial product or company? Yes/No. 
If yes, please provide details: "

In the following you will find different forms for the assessment of the outcome of your activity. The questionnaires listed are only examples and should be adapted to your specific event.

They can be used as interim (daily) or final evaluations and include the elements of visualization of the results on moderation walls or flipcharts. These methods are most effective in intensive workshop settings in smaller groups (max. 20) with much personal involvement.

The feed-back form on short interactive sessions [DOC,162KB]  using smileys is a very short visually attractive evaluation form.

The post-evaluation form [DOC,164KB] can be used to assess longterm outcome a certain time after the course.
